WELCOME! We are thrilled that you are interested in joining our 2015 Boot Camp Fitness Program. To complete the registration process, you will be required to complete the following:
(1) Physical Activity Readiness Questionnaire (PAR-Q) - This form is to ensure you are physically able to participate in a fitness program.
(2) Client Information Form - This form is to ensure that Prosperity Fitness has full knowledge of your health condition to ensure the exercise
program will be designed to meet all of your needs.
(3) Submit Payment - This is a four week fitness program, ranging from low to high intensity exercises. The fitness program will include a weekly weigh-in and body fat assessment. Additionally, meal modification advice will be shared. The cost is $60.00 per participant. No fees will be prorated.
